The short answer
Yes for any deck attached to the house. A freestanding deck needs one if it's bigger than 10 m² and more than 61 cm above the ground. City fee: $220.

The Town's sample drawings show footings at least 4 ft below grade, with deck blocks only for small freestanding decks. A $100 pre-screening fee is paid when you apply and credited to the permit.
What an application usually includes
Oakville’s checklist has the exact list; apply through Town of Oakville Online Services.
- A site plan with the lot lines, the house and the deck, with distances
- A framing plan: joists, beams, posts and footings with sizes and spacing
- Elevations and sections: guard, stairs, the ledger connection and footing depth
- The application form and the owner or designer declaration
Local quirks
- The Town's deck page and its permit checklist word the deck rule differently. Treat an attached deck as always needing a permit.
- Private trees 15 cm or more across need their own permit unless a building permit requires the removal.
- Grading work needs a development engineering permit unless it stays 0.3 m from the property lines and doesn't change drainage.
